Use the forms listed below as a checklist to ensure you and your students have everything needed prior to attending Shady Creek Outdoor School. The forms that are not available are being updated and will be sent to the schools as soon as possible.

Forms are available in English and Spanish (Punjabi coming soon). Please be sure that parents fill out the forms on the English version.

SPECIAL MEAL REQUESTS

If you have students who require special meals due to a medical condition, please give the “Medical Request for Meal Accommodations” to the students parents. Once completed, please scan and email to Kindy McCullough.
