Teacher Forms
Forms are in the process of being updated. Use the forms listed below as a checklist to ensure you have everything you need. The forms that are not available are being updated and will be sent to the schools as soon as possible.

The following forms you will need to send home with your students for completion:

These forms are available in English, Spanish and Punjabi. Please be sure that all parents fill out the forms on the English version.
If you have students who require special meals due to a medical condition, please give the “Medical Request for Meal Accommodations” to the students parents. Once completed, please scan and email to LorenzoC@sutter.k12.ca.us
