Use the forms listed below as a checklist to ensure you and your students have everything needed prior to attending Shady Creek Outdoor School. The forms that are not available are being updated and will be sent to the schools as soon as possible.

Forms are available in English and Spanish (Punjabi coming soon). Please be sure that parents fill out the forms on the English version.

SPECIAL MEAL REQUESTS

If there is a family with concerns about meals or their student's diet, please reach out to Lynsey "Yarrow" Brydon, Shady Creek Program Coordinator, at lynseyb@sutter.k12.ca.us or (530)822-2478. Lynsey can provide the family with a meal accommodation form if needed and work directly with the family to best accommodate the student.

Please feel free to share Lynsey's contact information directly with family who have concerns.
